Get a $50 incentive when you upload your COVID-19 vaccination record card online
Polytechnic University will award a $50 incentive to students who submit evidence of their COVID-19 Vaccination Record Card, by completing a vaccination validation form, no later than August 31, 2021. This form is available at MyPoly where you can upload your vaccination card. The information provided will be kept confidential. You must be registered in the student portal in order to access the vaccination validation form. The $50 incentive will be credited to your student's account for the FALL 2021 term.
